JADA CARES is a patient safety initiative of The JADA Foundation, Inc., dedicated to reducing preventable harm associated with compounded and non–FDA-approved medications.

 

Compounded medications play an important role in patient care, particularly when individualized treatment options are needed. However, these medications can also present distinct safety considerations that are not always well understood by patients or caregivers.

 

JADA CARES exists to support patients, caregivers, and communities by promoting education, encouraging responsible reporting, and fostering early awareness of potential safety risks—empowering individuals to make informed decisions and advocate for safer care.

JADA CARES

The JACA CARES Severity Tier system categorizes pharmaceutical regulatory activity into four tiers using documented regulatory actions and oversight conditions as comparative indicators. The system utilizes 26 standardized attributes, providing a foundation for consistent comparison and trend analysis.

 

The Severity Tier Overview supports pharmaceutical-sector governance analysis, public health and safety planning, and evidence-informed approaches to continuing education, workforce development, and public safety.

TIER 0
Corrective Actions

 Outcomes with minimal regulatory controls, such as education or training requirements. These matters typically involve a limited scope and no ongoing restrictions on practice.

TIER 1
Limited Enforcement

Outcomes involving modest sanctions, such as administrative fines or targeted corrective actions, without long-term supervision or practice restrictions.

TIER 2
Enhanced Oversight

Outcomes in which outcomes impose heightened oversight, such as probation, inspections, or operational conditions, indicate broader compliance or process concerns.

TIER 3
Intensive Regulatory Intervention

Outcomes involving significant and sustained regulatory controls, such as practice prohibitions, external compliance monitoring, long-term probation, or coordination with federal authorities.
